This waiver allows Cleveland Furniture Bank (CFB) or Caseworker to make furniture selections on behalf of a client.

NO WARRANTIES

By signing this waiver, you have agreed to allow your Caseworker or Social Worker, or an employee of CFB to select the furniture you will be providing through CFB’s Furniture for Families program. All furniture selected, with the exception of that provided to qualified clients through CFB’s Beds for Kids program, is known to be used furniture, in good condition. At the time the furniture is selected, items requested on the Voucher will be selected to the extent they are available. No guarantee can be made that all of the items will be available at the time of selection, and items not available may not be selected at a later date. If the furniture selected is to be delivered to the address shown on the voucher, the delivery fee must be paid in advance if not being paid by the client’s agency, and is not refundable. If you refuse the furniture selected for you at the ime of delivery, or you are not there at the scheduled time of delivery, you will be required to pay another delivery fee for re-delivery.

INDEMNIFICATION

Client, for successors and assigns, further agrees to indemnify and hold CFB free and harmless from any and all liability arising out of use and transportation of the described property.
